Adult Services Early Help Approach. PDF 199 KB Decision: Noted. Minutes: The Cabinet Member for Community (Support) introduced the item and outlined the context of the report. The Interim Director of Social Services then presented the report to inform the Social Care & Tackling Poverty Service Transformation Committee of the Prevention and Early Help strategic approach in Adult Services and contributing to next steps of the development of this approach. She provided additional context around the current demand for social care whereby demand at present significantly exceeded the resources available. As a result, a different approach in terms of adult services and supporting people earlier to maintain dependence to reduce or delay the need for managed care was outlined in section 2 of the report. Comments from the Committee focussed on Communication and the importance of getting this right for our communities and those in need of care. A workshop was suggested in order to discuss the different communication methods currently used. Councillor Gwilliam also highlighted the “one stop information shop” that was held on the first Monday of every month at the Waterfront Museum, however transport, costs and engagement barriers were highlighted. The Chair thanked the Officer for the informative report. Resolved that the report be noted. |

Transition (Child to Adult). PDF 142 KB Additional documents: Decision: Noted. Minutes: The Head of Child & Family Services presented the development of our services in relation to the transition from Child and Family Services to Adult Services for children with disabilities or additional learning needs. The report outlined the commitment, vision and ambition to support positive transitions from child and family services into adult services for children with disabilities or additional learning needs of our young people to inform our practice, and how we will develop and adapt our service as required to ensure these needs are appropriately met. The Committee commented on the excellent work being undertaken by the teams involved in the processes outlined. Resolved that the report be noted. |

Swansea Poverty Truth Commission - Rehumanising Systems and Services Charter. PDF 122 KB Additional documents: Decision: For information. Minutes: The Tackling Poverty Development Manager presented a “For information” report which provided the background and current situation in relation to the Swansea Poverty Truth Commission – Rehumanising Systems and Services Charter. The Committee appreciated the work undertaken by the Poverty Truth Commission and stated it was a great illustration of a group of people working together as equal investors in order to treat everyone with dignity, fairness, respect and compassion without judgement or discrimination. |

Minutes: The Child Disability Team Manager, assisted by the Commissioning Team Leader on behalf of the Principal Officer - Adolescent and Young People Service, presented a “For information” report to provide an overview of the provision by Child & Family Services of short respite breaks for both children and young people with disabilities and / or additional needs and their parents / carers. The Committee expressed their appreciation to the teams on the work undertaken in assisting families with short respite breaks. |
