Transition - How Does Transition Currently Work Across the Health, Social Care and Education System, Particularly Focusing on Citizen Experience. (Presentation)

Minutes:

The Principal Business Development and Commissioning Lead accompanied by the Child & Family Services Project Manager, provided a presentation on Transition – How does transition currently work across the Health, Social Care and Education system, particularly focussing on citizen experience.

 

He outlined the different types of Transition:

 

                  Care leavers and other vulnerable young people;

                  Young people moving from Child & Family Services to Adult Services and the process for moving from one to the other;

                  From compulsory education;

 

The legislative and policy drivers were explained and the suggestion for the PDC to focus on Learning Disabilities due to:

 

                  The most recent legislative and policy drivers would change our responsibilities to this group.

                  Our existing arrangements have not been systematically reviewed for a long time, e.g. the transfer protocol between Child and Family Services and Adult Services is dated 2011.

                  There is no specific multi-agency forum for discussing child disability and transition issues. Officers have recently been considering the formation of a new strategic group (the Local Offer Group) and it would be valuable to give it a clear mandate and purpose from the PDC.

                  Packages of care to support individuals with a learning disability are typically among the most expensive. This is true for Child and Family Services, Education and Adult Services.

 

He explained what we already know about the views of young people and families in these situations and provided details regarding the Children’s Commissioners findings in her report on transition “Don’t Hold Back” regarding:

 

                  Commissioners findings on social isolation;

                  Commissioners findings on the voice of the young person;

                  Commissioners findings on parental views;

                  Commissioners findings on the views of young people;

                  Commissioners findings on what parents want.

 

The Committee then discussed:

 

                  What the scope of their work should look like?

                  What cohort of young people should we focus on?

                  What end product and outcomes would we like to see?

 

Comments from the Committee consisted of:

 

                  The barriers to the changes that need to be implemented;

                  Ideal opportunity for the Committee to become involved in helping to shape the updated protocol;

                  As part of the review, we could also consider delivering training, emphasise the process for those reaching 18, then post 18, consider extending the Independent Advocacy facility for all young people unless they specifically state they do not want one and review the process annually.

                  Clarification that the Children’s Commissioners report was a pan Wales report and it was not known what % of the comments received related to Swansea;

                  Children’s Services do meet with Adult Services but may want to consider inviting colleagues from Health, CAMHS, Education, etc, however Adult Mental Health teams in the Health Board can’t participate in the planning stages until a child is 17 years and 9 months (due to guidance), which can be a problem for our Adult Services when they are trying to plan for a young person’s future.  Discussion have commenced in order to try to assist with this issue;

                  Extensive consultation was carried out with parent / carers  & children / young people last year but it was not specific to “Transition”;

                  The views of the children / young people vary to those of the parent / carers – they have different priorities;

                  Therefore, it is important to get the views of both the parents and children / young people of what they want in this specific process which should be reflected in any revised protocol;

                  Some initial discussions have taken place at the Parent Carer Group and comments echoed those outlined in the Children’s Commissioners report,  In addition they wanted to be part of the assessment and planning process;

                  Some vulnerable children / young people can be isolated in society;

                  Some parent / carers are worried about what will happened “after me”;

                  Whether we are in a position to include those with transgender issues?  There is not much support at present;

                  Consider whether there is learning / good practice that could be drawn from other Local Authorities.

 

Resolved that:

 

1.               The Chair meet with the Principal Business Development and Commissioning Lead and the Child & Family Services Project Manager to further consider the scope of the work;

2.               The cohort of young people could focus on mid teens to mid twenties;

3.               The end products and outcomes could include:

a.               Consultation via a “Swansea perspective” be undertaken with children / young people and parent / carers individually and together – so we are focusing on the “Citizen Experience” aspect;

b.               Review the Children / Young People with Learning Disabilities Transfer Protocol & Terms of Reference between Child & Family Services and Adult Services;

c.               A representative from the Health Board be invited to sit on the multi-agency group.
